L’organizzazione comprende i quattro mandamenti di Alcamo, Castelvetrano, Mazara del Vallo e Trapani, che raggruppano diciassette famiglie. Inevitabile il riferimento al latitante più ricercato, ritenuto il capo in assoluto e il leader più carismatico del mandamento di Castelvetrano, con una complessiva influenza anche nel palermitano.
La relazione si sofferma poi su alcune operazioni con le quali sono stati arrestati diversi collaboratori e prestanomi di Matteo Messina Denaro e sequestrati beni, complessivamente per centoventicinque milioni di euro e bloccate sette ditte alle quali non è stata concessa la certificazione antimafia per il sospetto di inquinazioni mafiose. Per il resto si elencano i reati più diffusi, come l’estorsione, ovvero il pizzo, l’usura, lo smercio di droghe, soprattutto hashish e marijuana, della quale sono state scoperte alcune coltivazioni.
Viene infine citato il sequestro di beni a un imprenditore che non viene citato: si tratta di Giuseppe Ruggirello, al quale sono stati sequestrati 25 milioni di euro, un vasto patrimonio comprendente villette, un lussuoso natante, esercizi commerciali, depositi bancari ecc. Non si parla naturalmente dei rapporti tra mafia e politica, molto diffusi nel trapanese, né della mafia dei colletti bianchi annidata nelle pubbliche amministrazioni. Tutto nella norma.
